We may use or disclose your information to obtain payment for your pharmacy services. For example, we may provide information to your health plan to determine eligibility or coverage or bill your insurance company for your prescriptions.

Health Care Operations

We may use or disclose your information to support our business activities such as quality assessment, employee review, training, and compliance audits.

Other Permitted Uses and Disclosures

We may disclose information when required or permitted by law, such as for public health reporting, law enforcement purposes, health oversight activities, or to prevent or lessen a serious threat to health or safety.

Other Uses and Disclosures Requiring Your Authorization

We will not use or disclose your health information for marketing, sale of information, or other non-routine purposes without your written permission. You may revoke your authorization at any time in writing.

Your Privacy Rights

You have the right to inspect and copy your health information, request an amendment, receive an accounting of disclosures, request restrictions on certain uses, request confidential communications, and receive a paper copy of this notice.

California-Specific Rights

Under California law (CMIA), your health information is also protected by state privacy regulations, which may provide additional protections for HIV test results, mental health or substance abuse information, and genetic test results.

We reserve the right to change this Notice and apply the new practices to all health information we maintain. Updated notices will be available in our pharmacy and on our website.
